Failing to do that as required by paragraph 3 above

  1. and imposed the following sentence(s) for those breach(es)

  1. Accordingly it was ordered that(2) Mr Gerard M O’Dwyer be committed for contempt to Her Majesty’s Prison at(3)      for a (total) period of              or until lawfully discharged if sooner, [and that a warrant of arrest and committal be issued forthwith].
    OR
    be committed for contempt to prison for a (total) period of 14 days
    The order is suspended until 12th December 2016 at 10:30am and will not be put into force if during that time Mr Gerard M O’Dwyer complies with the following terms:i) Completes and files a properly completed N56
